Commuters who were present as the Elphinstone stampede was taking place put blame squarely on the railway authorities for the tragedy.
According to eyewitnesses, a confluence of factors contributed to the stampede. One, that security guards were nowhere near the area, nor did they make their way there when the stampede was happening. Two, that the one bridge is used by passengers both getting on and off the train, leading to massive overcrowding.

Three, that hawkers have set up on the bridge that is meant for commuters, taking up space and adding to the crowding. Add to that, one commuter says, that the bridge is much too small to take the load of thousands of daily commuters, and you have a disaster in the making.
